Background

Dr Hao Pham and Mrs Annette Pham moved to the Shoalhaven in 1999, initially setting up a medical practice in Vincentia. Identifying a community need for accessible care Dr and Mrs Pham set about fulfilling the mission statement of Shoalhaven Family Medical Centres.


Today, Dr Hao Pham and Mrs Annette Pham oversee five general practices all located in the City of Shoalhaven.
Basin View Medical Centre, Culburra Beach Medical Centre, Vincentia Medical Centre, Ryan House Medical Centre and Worrigee Medical Centre.
All of these centres provide bulk billed primary health care services to a rapidly growing Shoalhaven community and have done so for over 17 years. 

Collaborative Team Approach

A collaborative health care team works at all centres and is made up of general practitioners (GPs), practice nurses (PNs) including an early childhood nurse, home assessment nurse, mental health nurses, a credentialed diabetes educator, a specialist Aboriginal health nurse, a podiatrist, an external pathology service provider, a visiting psychiatrist, a visiting dietitian, a practice administration team, and a senior management team, who together provide extensive primary health care services under one roof.
Proactive health practitioners identify preventative and chronic disease care concerns, address risky lifestyle behaviours, domestic violence, suicide risk, identify falls risk and refer clients for follow-up services within the practice and the community.
